Evidence-Informed Strategies for Service Coordination in Home Visiting and Collaborating Services

Early Home Visiting is a preventive strategy to build family assets and reduce risk for child abuse and neglect. Home visitors play a critical role in systems of care by providing direct parenting support and by linking families with needed services in their communities, such as income, housing, and nutrition assistance; physical, behavioral, and oral health services; employment training and education; child welfare; early intervention; and early care and education. Thus. home visiting programs must collaborate and coordinate with an array of health and human service agencies to ensure efficiency, prevent duplication of services, reduce unmet family needs, and achieve intended outcomes. This presentation will share foundational work conducted by the Jackie Walorksi Center for Evidence-Based Case Management, a federal initiative to identify, evaluate, and disseminate promising and evidence-based case management practices for home visiting. The team will review findings from a scoping literature review, environmental scan, and active engagement with key interest holders, such as state home visiting leads, national home visiting models, home visiting program staff, and families enrolled in services. Attendees will gain a deeper understanding of conceptualizations and components of case management and service coordination, barriers and facilitators to coordination, and promising strategies to ensure families are able to access community services that align with their needs and preferences. By synthesizing evidence and insights from multiple sources and sectors (e.g., health, social services), we will demonstrate potential for impact within and beyond home visiting. We will also share next steps for the Case Management Center’s work, including plans for knowledge development, professional development, and technical assistance.
